Vasoactive Intestinal Peptide (VIP) Nasal Spray Description1-6

Vasoactive intestinal polypeptide (VIP) is a 28 amino acid peptide. It is a neuropeptide that works as a neurotransmitter in the peripheral and central nervous system. It has activity as vasodilator, impacts smooth muscle activity (especially in the respiratory tract and gastrointestinal tract) and influences blood flow in the gastrointestinal tract. The spray is compounded into a nasal spray to provide delivery into the central nervous system and absorption into the systemic circulation. Clinically, it has been used for chronic inflammatory response syndrome, respiratory diseases including asthma, chronic obstructive pulmonary disease, cystic fibrosis, pulmonary hypertension, and sarcoidosis.

Mechanism of Action1-6

Vasoactive intestinal polypeptide (VIP) is found in the peripheral, gastrointestinal and central nervous systems. It is produced by immunomodulatory T cells. Studies have shown that VIP is in nerve fibers in the following organ and systems: gastrointestinal tract, respiratory airways, pancreas, sensory organs, and reproductive system. It binds to two receptors VPAC1 and VPAC2. VIP receptors are found in various tissues in the gastrointestinal tract and brain. Receptors are specifically found on smooth muscle of sphincters of the lower esophagus, ampulla of Vater, and rectum. VIP regulates sphincter relaxation. VIP receptors on pancreatic acinar and duct cells and enteric mucosal cells mediate fluid secretion in these organs. In the lungs, VPAC1 is found in the lung tissue and VPAC2 is essentially found in the lung smooth muscles.

VIP activates adenylate cyclase and increases cyclic AMP (cAMP) levels. This activity of VIP may have the following activities: anti-inflammatory (immune modulation), vasodilation (enhances blood flow and reduces vascular resistance especially respiratory tract), neuroprotection, and regulation of smooth muscle to help with motility and digestive health.

  • Known intolerance or hypersensitivity to VIP or inactive components in nasal spray

Precautions1-6

  • Patients with hypotension
  • Patients with cardiac disease

Store VIP nasal spray in a refrigerator at 2°C to 8°C (36°F to 46°F). Do not freeze. Protect from light and keep the bottle upright. Discard any unused portion after the expiration date indicated on the label.
