Compounded medications are “made from scratch” – individual ingredients are mixed together in the exact strength and dosage form required by the patient to better meet their needs. History of USP <797> for Patient Safety

USP <797> was created out of a need to standardize the way sterile medications were prepared in pharmacies. USP is a non-governmental agency, which has set chemical and process standards for more than 100 years. They first introduced USP <797> in 2004 as a guideline for pharmacists. Over the last 10 years, pharmacy boards around the U.S. have adopted the standards as law. USP <797> has been updated several times, most recently in 2012.
