Semaglutide Description1-6

Semaglutide, a synthetic glucagon-like peptide-1 receptor agonist (GLP-1 RA), is an incretin mimetic. Incretins are released from the gastrointestinal tract into the systemic circulation to improve glycemic control and have been used to aid in weight loss. Orally, the drug has been used for the following indication:

  • For the treatment of type 2 diabetes mellitus as an adjunct to diet and exercise

Evidence suggests that oral semaglutide can provide clinically meaningful results for patients who would like to lose weight. In a 68-week study, oral semaglutide was shown to decrease weight by 15%.5

 

Metformin Description7-9 

Metformin is an oral biguanide used to improve glycemic control in adult and pediatric patients 10 years and older with type 2 diabetes mellitus (T2DM). The drug is often 1st line for patients with T2DM. The drug is indicated for

  • For treatment of type 2 diabetes mellitus as an adjunct to diet and exercise

There is evidence that metformin facilitates weight loss and decreases the appetite of patients who are administered the drug.9

Semaglutide is an incretin mimetic that works on the glucagon-like peptide-1 (GLP-1) receptor by binding and activating the GLP-1 receptor. It has 94% sequence homology to human GLP-1. GLP-1 medications regulate glucose metabolism after oral consumption of carbohydrates and fats. The drug increases insulin secretion by glucose-dependent insulin synthesis and in vivo secretion of insulin from pancreatic beta cells in the presence of elevated glucose levels. It also suppresses glucagon secretion, slows gastric emptying, reduces food intake, and promotes beta-cell proliferation.

 

Metformin7-9 

As an antidiabetic medication, metformin reduces gluconeogenesis production in the liver, decreases absorption of glucose in the gastrointestinal tract, and increases insulin sensitivity and subsequently peripheral glucose uptake and utilization. The drug facilitates glucose utilization in skeletal and adipose tissue. Metformin is believed to facilitate weight loss by decreasing caloric intake by patients through gastrointestinal adverse effects and appetite is decreased by production of glucagon like peptide 1 production.8,9

Semaglutide and Metformin Common Adverse Reactions 

Gastrointestinal: Nausea, vomiting, diarrhea, constipation, dyspepsia, headache, dizziness.

Contraindications & Precautions3,4,7-9

Contraindications:

  • Hypersensitivity to semaglutide or metformin or its components
  • In people with a personal or family history of medullary thyroid carcinoma (MTC) or in people with multiple endocrine neoplasia syndrome type 2 (semaglutide)
  • In patients who are pregnant
  • In patients with chronic metabolic acidosis, including diabetic ketoacidosis (metformin)

 

Precautions:

  • In patients with a history of pancreatitis and gall bladder disease (semaglutide)
  • In patients with renal and hepatic impairment

Store at 20°C to 25°C (68°F to 77°F). Protect from light, moisture, and heat.
