Exemestane, a steroidal aromatase inhibitor that is structurally similar to androgens (androstenedione and testosterone), irreversibly inhibits estrogen production. It is used in postmenopausal women for advanced breast cancer and for estrogen-receptor positive early breast cancer. [1,2]
Aromastase activity is irreversibility inhibited by exemestane. It reduces estrogen (estrone, estradiol and estrone sulphate) levels by approximately 90%. The enzyme aromatase is critical in converting androgen from the adrenal gland and ovaries to estrogens. The drug is structurally like androgens which provides the attraction for binding to aromatase. The process of binding leads to “suicide inhibition” of the drug when binding irreversibly to the enzyme. With less estrogen production hormone-dependent breast cancer cells regress. [1,2]
