Product Overview

Benzocaine_Lidocaine_Tetracaine (BLT) Cream, a mixture of 3 anesthetics, is a compounded topical cream used before dermatologic procedures such as laser therapy, tattooing, microneedling, and dermal filler injections. Anesthetics used locally on the skin are grouped into either amino amides or amino esters. Lidocaine is an amino amide while both benzocaine and tetracaine are amino esters. Benzocaine has an onset of action of about 1 minute and action lasting about 15—20 minutes. Tetracaine has an onset of action that ranges between 3 to 10 minutes and the duration of action of 30 to 60 minutes. Lidocaine has an onset of action of approximately 1 to 2 minutes, and duration of action of approximately 15 minutes. The combination offers rapid onset and prolonged numbing effects by leveraging each component’s unique pharmacokinetic profile. Both benzocaine and tetracaine are ethyl ester of p-aminobenzoic acid.1-5

The mechanism of action for these local anesthetics of benzocaine, lidocaine and tetracaine is to reversibly block voltage-gated sodium channels. They prevent sodium from moving into the nerve cells and stop the impulse transmission. These drugs block sodium in a use dependent manner meaning that the higher the firing rate, the more efficacious the drug is in blocking the sodium channel.1-5

  • Temporary numbness or tingling

Contraindications & Precautions

Contraindications: 1-5

  • Known hypersensitivity to benzocaine, lidocaine, tetracaine, or PABA derivatives
  • Use on broken or inflamed skin increases systemic absorption and risk of toxicity

Precautions: 1-5

  • Avoid occlusive dressings over application sites to reduce systemic absorption
  • Monitor pediatric patients and the elderly more closely due to increased sensitivity

Store at 20–25°C (68–77°F) in a dry place, protected from light and heat [8]. Do not freeze. Keep out of reach of children.
